The Andover Health Division regularly inspects restaurants and stores that serve and sell food. Andover News reviewed reports from health inspections conducted during the month of November.

It’s important to note it’s not unusual for a restaurant to have a few minor infractions in a report.

“The reports do not always reflect the nuances of the regulations, and very few establishments will have ‘no violations’,” Andover Public Health Director Thomas Carbone said last year, when we started posting inspection reports. “Our inspection program is designed to assist the establishments in providing a safe product for their customers; compliance with the food code is the best way to ensure that safety.”

Andover News publishes reports monthly. Look for January’s inspection reports in early February.

The reports below are broken into three categories: restaurants, stores and other establishments, which can include schools, daycare centers, nursing homes, hotels and other facilities that serve food.


Restaurants

The Chateau Restaurant, 131 River Road, RE-INSPECTION on Dec. 6: 0 violations, 0 repeat violations.

Genji Sushi, 40 Railroad Street, on Dec. 14: 1 violation, 0 repeat violations.

  • Observed distilled water used for testing pH of sushi rice to be outdated upon arrival to inspection.
  • Discussed with PIC to post CFPM and allergen certificates in public view adjacent to copy of food permit.

Grassfields Food & Spirits, 207 North Main Street, RE-INSPECTION on Dec. 5: 0 violations, 0 repeat violations.

La Fina, 27 Main Street, on Dec. 22: 3 violations, 0 repeat violations.

Mootone, 15 Railroad Street, on Dec. 5: 1 violation, 0 repeat violations.

  • CONSUMER ADVISORY OBSERVED TO BE INCOMPLETE, PIC STATES INTENTION TO MAKE EDITS TO CONSUMER ADVISORY WHEN NEW MENUS ARE MADE. WILL VERIFY AT NEXT ROUTINE INSPECTION.
  • Observed ample amounts of grease on side of fryer, on floor behind fryers, and under grill in kitchen area.
  • Observed food permit posted in public view with in expiration date in “2021.” PIC to obtain current copy of food permit and post in public view.

Palmer’s Restaurant & Tavern, 18 Elm Street, RE-INSPECTION on Dec. 6: 2 violations, 0 repeat violations.

  • NO TEMPERATURE INDICATOR AVAILABLE AT TIME OF RE-INSPECTION. REVIEWED OPTIONS OF INDICATORS WITH PIC. PIC TO SEND ORDER FORM OR RECEIPT TO HEALTH OFFICE VIA EMAIL WHEN INDICATOR IS PURCHASED OR ORDERED.
  • PIC STATES INTENTION TO MAKE EDITS TO CONSUMER ADVISORY WHEN MENUS ARE EDITED. WILL VERIFY AT NEXT ROUTINE INSPECTION.
  • No covered receptacle available in unisex toilet room at time of inspection for the proper disposal of sanitary napkins. PIC to provide covered receptacle in toilet room.
  • Observed allergen certificate with an expiration date of “1/7/2022.”
  • Discussed with PIC to post CFPM and allergen certificates adjacent to food permit in public view.

Sebastian’s Cafe, 20 Brickstone Square, RE-INSPECTION on Dec. 5: 0 violations, 0 repeat violations.

Smythe & Dove Steak, 89 Main Street, on Dec. 22: 2 violations, 0 repeat violations.

  • No disposable paper towels available in unisex employee toilet room at time of inspection.
  • Observed both low-temperature warewashing machines in establishment not dispensing a detectable amount of bleach sanitizer after being run for multiple cycles.
  • Observed ice machine with accumulation of what appears to be mold around chute.
  • No bleach sanitizer testing strips available to test low-temperature warewashing machines at time of inspection.
  • Observed expired quaternary sanitizer testing strips at time of inspection.
  • No covered receptacle available in unisex employee toilet room for the disposal of sanitary napkins.
  • No allergen awareness training certificate available at time of inspection.
  • Discussed with PIC to post CFPM and allergen certificates in public view adjacent to food permit.

Tokyo Steakhouse, 560 South Main Street, on Dec. 5: 5 violations, 0 repeat violations.

  • No handwashing signage available at handwashing sink at sushi bar or at bar.
  • Observed handwashing sink behind bar to have empty beverage containers being stored directly inside of it.
  • No parasite destruction letter on site from fish supplier stating whether fish used for sushi have either undergone parasite destruction or are farm-raised, therefore not requiring parasite destruction.
  • Observed large ice machine with what appears to be mold around chute area.
  • Observed fried shrimp and fried sweet potato patties sitting out of temperature control near fryers upon arrival to inspection.
  • Observed two unlabeled chemical spray bottles with chemical that PIC identified as sanitizer not labeled with common name of chemical it contains.
  • No sushi rice lab testing results available on site. PIC to ensure sushi rice is tested yearly to ensure compliance with HACCP plan.
  • Observed distilled water used for testing pH of sushi rice to be outdated.
  • Observed containers of in-use utensils being stored in water on flat-top grill below 135ºF.
  • Observed several boxes and containers of foods being stored on floor of walk-in freezer.
  • Observed expired quaternary sanitizer testing strips at time of inspection.
  • Observed multiple warewashing racks with significant discoloration and in general poor condition.
  • Observed mesh screen on screen door to be in poor condition with large gap present on top.PIC states steaks on menu are cooked to customer preference. PIC to add asterisks to steak items to indicate they are applicable to consumer advisory already present on menu.
  • PIC to replace batteries in dish plate thermometer to accurately test high-temperature warewashing machine.
  • Due to multiple outstanding violations regarding establishment’s HACCP plan for sushi and per discussion with health office, establishment is to cease and desist sushi operations until able to provide evidence to health office that requirements have been met.

Yella, 16 Post Office Square, RE-INSPECTION on Dec. 6: 0 violations, 0 repeat violations.


Stores

Stop & Shop, 209 North Main Street, on Dec. 14: 2 violations, 0 repeat violations.

  • Observed open reach-in refrigerator containing commercially pre-packaged hams at 45ºF. PIC states hams have remained in refrigerator since the beginning of this week. All products were taken out of refrigerator unit during time of inspection.
  • Observed multiple opened bulk packages of deli meats in refrigerator behind deli counter missing “use-by” dates. PIC states meats remain in refrigerator over 24 hours. PIC voluntarily discarded unlabeled deli meats at time of inspection.
  • Observed expired food permit at time of inspection.
  • Discussed with PIC to post CFPM and allergen certificates in public view adjacent to food permit.

Whole Foods Market, 40 Railroad Street, on Dec. 14: 1 violation, 0 repeat violations.

  • Observed multiple bakery items packaged in a way that does not protect food from potential contamination and tampering. PIC to ensure foods are packaged in a way that does not pose opportunity for potential contamination and tampering.
  • Discussed with PIC to post CFPM and allergen certificates in public view adjacent to copy of food permit.

Other Establishments

Chinmaya Mission Boston, 1 Union Street, on Dec. 5: 0 violations, 0 repeat violations.

Lanam Club, 260 North Main Street, on Dec. 14: 0 violations, 0 repeat violations.

Marland Place (Main Dining), 15 Stevens street, RE-INSPECTION on Dec. 5: 1 violation, 0 repeat violations.

  • OBSERVED CONTAINERS OF SLICED TOMATOES AND HARD-BOILED EGGS BOTH IN PAN IN TOP SECTION OF IN-LINE REFRIGERATOR AT 54ºF AND 51ºF RESPECTIVELY. EMPLOYEE STATES TOMATOES WERE SLICED YESTERDAY. BOTH FOOD ITEMS WERE VOLUNTARILY DISCARDED AT TIME OF RE-INSPECTION.

Marland Place (Main Dining), 15 Stevens Street, RE-INSPECTION on Dec. 22: 0 violations, 0 repeat violations.

Nexdine, 95 Old River Road, on Dec. 5: 0 violations, 0 repeat violations.

Pfizer Cafe, 1 Burtt Road, on Dec. 6: 0 violations, 0 repeat violations.
