The Andover Health Division regularly inspects restaurants, stores, schools, and other establishments that serve and sell food, and Andover News publishes the inspections reports monthly. It’s important to note it’s not unusual for a restaurant to have a few minor infractions in a report.

The reports below are broken into four categories: restaurants, stores, schools, and other establishments, which can include daycare centers, nursing homes, hotels and other facilities that serve food.


Restaurants

Akita Ramen, 199 North Main Street, on Feb. 25
Areas where the establishment was out of compliance:

  • Observed multi prepped meats with no date marking.
  • Observed Concentration of sanitizer is too strong.
  • Observed multiple pieces of equipment not in use.
  • Observed back door not properly sealed, with multiple entry points for pests.

Andover Deli Market, 22 Andover Street, on Feb. 21
Areas where the establishment was out of compliance:

  • No hot water present in all the sinks.

Breadwinner’s Bake Shop, 6 Alpine Drive, on Feb. 4
Areas where establishment was out of compliance:

  • None.

Good Day Cafe, 165 Haverhill Street, on Feb. 3
Areas where establishment was out of compliance:

  • Observed rodent droppings under prep table next to stove @ back prep kitchen.
  • Observed missing ceiling tiles.
  • Update allergen awareness sign to include all major allergens.

Karma, 209 North Main Street, on Feb. 27
Areas where the establishment was out of compliance:

  • Observed no hand drying provisions in hand sinks located at warewashing area and cook line.
  • Observed veggies stored under raw chicken and lobster in walk in.
  • Observed multi containers of food on walk in and freezer floors.
  • Food was observed stored directly on the floor.
  • Observed rodent droppings near back door and mop sink.
  • Observed grease and food debris under fryolators in appetizer prep station.
  • Observed double door refrigerator in appetizer prep area with rust and decay.
  • Observed hood crates with grease accumulation.

Kokoro Craft Udon, 12 Post Office, on Feb. 4
Areas where the establishment was out of compliance:

  • Repair floor so that it is sealed, cleanable, non-absorbent, does not allow water to pool. Flooring must be commercial grade material.
  • Observed prep sink used for hand washing and dish washing.
  • Observed the only hand sink located in kitchen obstructed and not functioning. The water had been turned off.
  • Observed rodent dropping behind bin holding brown sugar.
  • Observed hood filters with dust and food debris.
  • Observed microwave needing a cleaning.
  • Observed ice machine with grime.
  • Observed grease interceptor servicing cook line leaking.
  • Observed grease interceptor servicing the warewashing area not functioning, dirty and smelly.

Lulu’s Lunchbox, 195 Andover Street on Feb. 24
Areas where the establishment was out of compliance:

  • Upon entering establishment for inspection, no paper towels were observed at the handwashing sink.
  • Raw eggs stored above strawberries in reach in cooler.
  • Cooked chicken stored on bottom shelf below raw items and next to raw chicken.
  • No date marking observed on food items taken from original container and placed in reach in cooler.
  • No date marking observed on items placed in containers in display case.
  • Sanitizer observed above max concentration of 400ppm. PIC remixed sanitizer to proper concentration.
  • No ambient thermometer observed in cold holding display case.
  • Establishment did not have any test strips available for sanitizer during inspection.

Mootone, 15 Railroad Street, on Feb. 26
Areas where the establishment was out of compliance:

  • Observed hood inspection sticker 90 days past due for inspection.

Teatone, 17 Railroad Street, on Feb. 19
Areas where the establishment was out of compliance:

  • None.

Thai Sweet Basil, 209 North Main Street, on Feb. 25
Areas where the establishment was out of compliance:

  • Observed floor underneath cook line with grease and food debris, especially between grease interceptor and between equipment Thoroughly clean non-food contact surfaces to remove build up of debris/splash/grease.
  • Observed warewashing area with a bad odor and dirty floors, Thoroughly clean non-food contact surfaces to remove build up of debris/splash/grease.
  • Observed hood filters with grease accumulation. Inspection sticker was also past due, hood cleaning company is scheduled to come within the week.
  • Observed ceiling tiles near fryolator with water damage and some missing.
  • Observed delivery door dirty and in need.

Ultimate Perk, 96 Main Street, on Feb. 10
Areas where the establishment was out of compliance:

  • Food Employee Reporting Agreement form, and/or document of training observed during inspection.
  • Sanitizer tested above 400ppm. COS: PIC diluted sanitizer to 200ppm.
  • Gap observed under door in rear storage area, may allow for the entry of pests.

Stores

Restaurant Depot, 148 Dascomb Road, on Feb. 24
Areas where the establishment was out of compliance:

  • Observed hand sink located produce prep area, blocked and with food product.
    Observed hand sink at meat area, with no hot water, light water flow and broken.
    Observed dirty mop water and mop.
    Observed leaking ceiling, near eggs.

Sunday General Store, 195 Andover Street, on Feb. 24
Areas where the establishment was out of compliance:

  • None.

Schools, Churches & Daycare

No inspections in February.


Other Establishments
