Akita Ramen, 199 North Main Street, on July 26
Areas where establishment was out of compliance:
- Observed hand sink in front of the house-obstructed
- Observed hand sink in prep area-obstructed
- Observed rodent droppings in front of house under hand sink
- Observed multiple equipment not being used
- Observed back door with multiple unsealed areas
Bartlet Street, 11 Bartlet Street, on July 26
Areas where establishment was out of compliance:
- Hand sink located in kitchen not operating at time of inspection.
- Observed non compliant 2oz portion cups
Chili’s, 131 River Road, on July 30
Areas where establishment was out of compliance:
- Observed non compliant styrofoam
D’Agostino Deli, 89 Main Street, on July 24
Areas where establishment was out of compliance:
- Hand sink upstairs was blocked with items.
- Hand sink In basement was blocked by various items. Completely inaccessible to staff
- Staff observed wearing gloves when handling phone, register into between food prep tasks. Gloves were not changed and/ or hands were not washed between glove use
- Chicken observed left out on top of freezer.
- Cold holding temps in most units observed above limits.

Areas where establishment was out of compliance:
- Observed hand sink located in warewashing station with no hot water.
- Observed raw meats stored above vegetable
- Observed rodent droppings behind mixer on cook line and infront of walk in freezer door
- Observed various containers with food in walk in floor
- Observed ice machine with dirt debris
- Observed grease/ debris under fryolayor located in cook line and under multiple fryolayors in appetizer station

- No hot water in hand sink located in cooking area, hand sink leaking.
- Observed 2 cutting tables with deep cuts and food debris, need to be replaced
- Observed grease and food debris between and under equipment in cooking area
- Observed dripping grease in hood
- Observed ice machine with dirt debris

- Observed mold growth in cold holding unit storing meat scrape

- All vents located in produce prep area, dust accumulation
- Observed missing ceiling tile above dishwasher in bakery
