The Andover Health Division regularly inspects restaurants, stores, schools, and other establishments that serve and sell food, and Andover News publishes the inspections reports monthly. It’s important to note it’s not unusual for a restaurant to have a few minor infractions in a report.

Restaurants
34 Park, 34 Park Street, on Oct. 24
Areas where establishment was out of compliance:
- Observed hand sink located in cook line obstructed.
- Observed rodent droppings behind the host stand, in pizza prep area behind containers of bread crumb and semolina flour.
- Observed multiple flip top refrigerators with broken lids.
- Observed walk in door not self latching. External thermometer reading high but internal temps are correct.
Depot House of Pizza, 53 Essex Street, on Oct. 3
Areas where establishment was out of compliance:
- Marinating raw chicken and beef stored above ready to eat food items in single door, “superior” cold holding unit.
- No date marking system currently in place.
- Please have TPHC procedures on site for pizza and cooked chicken.
- Asterisks not on consumer advisory. Must correspond with food item that may be offered as undercooked.Unlabeled sanitizer bottle.
- Debris observed on can opener blade.
Genji Sushi, 40 Railroad Street, on Oct. 11
Areas where establishment was out of compliance:
- None.
Grassfields, 207 North Main Street, on Oct. 10
Areas where establishment was out of compliance:
- Employee observed touching face with hand before donning gloves on cookline. Employee instructed to remove gloves and wash hands before donning new gloves.
- Handwashing sink observed blocked by items stored in front and strainer.
- No signage at handwashing sink at bar.
- No hand drying provision at handwashing sink at bar.
- Build up of debris observed inside ice machine.
- Sanitizer bottle not labeled.
- Employee drink observed stored on cookline. Make sure employees have designated area to eat and drink that does not pose risk of contamination of food. Drinks should have lids.
- Sanitizer bucket stored in a manner which may contaminate food if spilled.
- Ambient water temp measured at 104*F.
- Ice build up observed on condenser fans and on shelving around door in freezer.
Monica’s Mithai Cafe & Bar, 4-16 Post Office Avenue, on Oct. 4
Areas where establishment was out of compliance:
- Food Employee Reporting Agreements were not available for inspector review. All employees must be informed in writing of their responsibility to notify the manager when they experience symptoms, diagnoses and exposures to Foodborne Pathogens and records must be kept of agreement on site.
- No Consumer Advisory was observed on menu/menu board. Add Consumer Advisory to menu and identify every item it applies to with asterisk
Raagini Indian Bistro, 209 North Main Street, on Oct. 7
Areas where establishment was out of compliance:
- The temperature of the Samosa chat was 122 degrees.
- The temperature of the Yogurt was 58 degrees. PIC made larger ice bath.
Sho Yu Restaurant & Bar, 36-42 Park Street, on Oct 24
Areas where establishment was out of compliance:
- Observed tempura batter flour located under hand soap dispenser next to hand sink.
- Observed brown rice with mold in walk in, no date marking.
- Observed rodent dropping throughout establishment.
- Observed hood crates with grease build up.
- Observed grease and food debris under cookline.
Stores
Whole Foods, 40 Railroad Street, on Oct. 11
Areas where the establishment was out of compliance:
- Observed cooling pastry racks next to hand sink with no splash guard and in from of 3 bay.
- Observed rodent droppings under bulk area.
- Observed debris and mold in produce prep area on walls, between pipes and handsink and outside of ice machine.
